Registration Procedure

Please provide ALL information required to your team representative so that a pass can be issued in a timely manner. You cannot play without a pass. PERIOD.

If you are a NEW player the registration kits require:

1. TWO copies of a completed and signed registration form.
2. One color wallet head shot photo with the player's name written on the back of the photo.
3. One photocopy of a photo ID (i.e., school ID, drivers license, state issued ID).
4. $30.00 registration fee (Your team may charge more for team dues, but your team rep must submit $30.00 for each player registered.

If you are a RETURNING player, all you need are TWO signed copies of the registration form submitted to your team rep.
